Patient Information
Carson Valley Oral Surgery
Your first appointment will be scheduled to provide you with the doctor’s undivided time and attention to ensure that your individual needs are being met and that your questions are answered.

This consultation will include a comprehensive examination with the necessary diagnostic x-rays, as well as information provided by your dentist, so that your situation can be thoroughly evaluated and the ideal treatment plan can be determined. If your dentist has taken x-rays recently, please request that they be forwarded to our office.

Following your examination, we will discuss the ideal treatment option and any alternatives, and explain the procedures involved and the treatment sequence. We will also show you photos of similar cases so that you can better understand the recommended treatment plan.

Occasionally, surgery can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:
  • Your surgical referral form
  • If your dentist or physician has taken x-rays, please bring them with you, unless there is enough time to have them forwarded directly to our office.
  • A list of medications that you are presently taking
  • If you have medical insurance or dental benefits, bring the necessary completed forms. This will save time and allow us to help you process any claims.
For your convenience, you can use the link below to complete your patient health and financial information, Patient Forms.

IMPORTANT REMINDER: All patients under 18 years of age must be accompanied by a parent or guardian.

A pre-operative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink for 8 hours prior to your surgery, if you are an adult, or as Dr. Gray directs, for pediatric patients. You will need to be accompanied by an adult who will remain in the office during the procedure, and who will drive you home after the procedure.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.), if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.), or if you have any allergies to medicines.

Scheduling Appointments
Carson Valley Oral Surgery is open from 8:00 to 4:30 Monday, Tuesday, Thursday, and Friday. We will schedule your appointment as promptly as possible. If you are experiencing pain or an emergency situation, every attempt will be made to see you the day you call.

We strive to schedule appointments that are convenient to your schedule. We respect your time and make every effort to see you on time. Due to the fact that we provide surgical services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.

So that we can maintain our schedule to accommodate all of our patients, we ask that you arrive 10-15 minutes prior to your appointment to complete all necessary paperwork. In addition, under the “Patient downloads” section of our website, you can print and complete the necessary forms prior to your visit. Just bring your completed forms with you.

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice.  This courtesy on your part will make it possible to give your appointment to another patient.
Financial Options & Insurance
Our primary goal is to provide you with the finest, leading edge oral surgery and dental implant procedures available today. We are concerned about you and your dental care and make every effort to ensure that you will be treated in the most responsible manner, with kindness and understanding. In order to assist you with your investment in your oral health, we have outlined the financial options available.

We ask all of our patients to take care of their fees on, or before, the day of their treatment. For your convenience, we accept cash, and credit card payments (MasterCard, Visa, Discover, American Express, and Care Credit).

We are a provider for Delta Dental, Diversified, Aetna, Dentamax, Preferred Dental and other major plans. If you are unsure if we are a provider for your plan please call the office. Remember, even if we are not contracted with your plan, your insurance may still provide strong coverage here.

As a courtesy to you, we will submit your claim forms. Please remember you are fully responsible for all fees charged by this office regardless of your dental benefits. Your dental benefits are part of an agreement you or your employer has made for payment of your care. This generally has little to do with the treatment you may need or the cost of that treatment.

We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility and your prompt remittance is appreciated.

If you have ay questions regarding payment arrangements for your initial visit, or treatment, please call our administrative staff at 775-782-6491.
